Right now since I only have 24 members on the team I have a short paper form that I made in MS word that is 5.5 X 8.5 size.  

It is attached as a PDF.

Our CERT team is part of a volunteer fire protection district. We have several organizations that requre record keeping. I have developed a Access database that maintains rosters for all of the organizations and keeps track of issued items, vacinations, training, etc. I will share it upon request. The only requirement is that my name as author be kept in tact.

I am trying to set up a data base for our CERT teams and would really like to see what you have done. I don't believe in reinventing the wheel so if you have something I can use with your name intact I am all over it. [smiley=thumbsup.gif]

    I would really like to use your program if at all possible.  I'm the EMD for the City of New Madrid, MO.  This would be a great asset for our CERT team.  These records could be stored on a computer, instead of just paper.  I would promise you that your name would be kept as author.  I would be willing to sign a consent form.  You can email it to me at emergencymanagement@new-madrid.mo.us  Thank you very much.

Color me interested. Starting a Citizen Corp in our community and I will be responsible for all of our folios, Neighborhood Watch, CERT, Fire Corps, Medical Corps and VIPS. Needless to say I will maintain your requirement. Thanks in advance.
